the licensor and service provider ("Service Provider"). The Web site gives authorized
users the ability to schedule appointments over the Internet with participating
health care providers. Your use of the Web site is governed by this privacy policy
of Service Provider and the Terms of Use Agreement posted on this Web site. As part
of the normal operations of this Web site, we collect and may disclose information
about you in connection with your use of this Web site. This Privacy Policy explains
the types of information we gather and what we do with it.
Information We Collect And How We Use This Information
Service Provider collects various types of data from you and other authorized users,
including basic information such as your name, your site or clinic name, address,
e-mail address, etc., which is gathered from you as you complete the information
required by the Eceptionist® scheduling application. This information will
only be shared with other authorized users of this Web site who have access rights
to this information. Service Provider will not share this information with other
third parties for marketing, analytical or any other purposes.
As part of the Eceptionist® scheduling application, we will also collect personally
identifiable information about the patients for whom appointments are scheduled,
including their names, social security numbers and medical conditions. No personally
identifiable information will be used or disclosed by Service Provider for any reason
unless the information is knowingly provided by the patient and the patient consents
to the disclosure for the purpose for which the information is collected at the
point of disclosure. For this purpose, Service Provider will be entitled to rely
upon the user's input of any and all information into the Eceptionist® scheduling
application, including verification that the patients have signed the appropriate
consent forms.
Service Provider and or Company also monitors usage of the Web site, including user
traffic patterns, page views and user sessions.
Our Use Of Cookies
As part of our customized services, Service Provider uses cookies to obtain and
store information about users of this Web site. A cookie is an Internet mechanism
that sends small pieces of information to your browser and stores it on your computer's
